Report Your Injury

Report the injury to your employer and obtain a workers' compensation claim number from the insurance carrier.

Schedule or Walk In

Call us or walk in with your claim information. Bring your employer's name, adjuster contact info, and claim number.

Get Treated & Return to Work

Our providers will evaluate, treat, and manage your care — including work status reports and return-to-work clearance.
