Access the athenahealth Patient Portal, an electronic means to obtain your medical records. If you have a Patient Portal account, you can access portions of your records at any time. If you are a patient and do not yet have an account, please create your Patient Portal account.
If you would like to request a copy of your medical records, radiology images, and/or billing statement, please complete the Authorization for Access/Use/Disclose Protected Health Information form below.
Once the form is completed, send it to Movement Orthopedics Medical Records Department:

If you are a third party (attorney, insurance company, etc.) and need to request medical records for a Movement Orthopedics patient, you may submit your request along with the patient's authorization one of the following ways:
